Functional Unit for Cardiac Failure

This is a multidisciplinary Unit within the Hospital, led by the Cardiology Service, which includes different Services based on treating patients with Cardiac Failure.

The Unit is located on floor 0 of building A, module 6.

 

Objectives of the Unit

Give high-quality care to patients, coordinating with the Primary Care Unit and facilitating access to tertiary services when required.
Reduce re-admissions and improve efficiency in the care process.
Improve care continuity and efficiency in attention through the analysis and definition of the care model.


Services portfolio:

Patients with cardiac failure.
Patients with acute decompensated heart failure.
Patients with refractory heart failure and assessment of palliative care.
Patients for the assessment of heart transplants.
Patients on the waiting list for a heart transplant.
Patients with heart failure who require an IAD implant.
Patients with left ventricular assist devices.
